Membership Label/Roster Service
Printed membership labels/rosters of individuals or firms are available to third parties for single-use rental only. Names and addresses are provided for mailings in either the label or roster format and firm telephone numbers are provided in the roster format only.
Individual Members
- $.28 per peel-off label (8.5 x 11 - 3 across 10 down)
- $.28 per roster name (8.5 x 11 - piece of paper)
Firms
- $.35 per peel-off label (8.5 x 11 - 3 across 10 down)
- $.35 per roster name (8.5 x 11 - piece of paper)
Ordering criteria
- Minimum charge for any order is $150.00.
- A deposit of $150.00 is required along with the submission of any order. Checks along with Visa, Master Card, American Express, and Discover credit cards are accepted.
- A copy of what will be sent or a script of what will be said before, is required to be provided before an order will be processed.
- E-mail addresses and label/roster information on disks are not available.
- There is a $4.50 shipping charge for all orders. Orders are sent UPS unless otherwise specified. UPS will NOT deliver to a PO Box.
- Tax exemption allowance is accepted. Please include your federal tax ID number with your order or a 6.75% Franklin County sales tax will be included.
- Purchasing a membership list is not commissionable by agencies.

- Orders/requests MUST be submitted in writing by utilizing the label/roster order form below. Please include specific criteria requested or order will be delayed.
- Please allow 10 business days for delivery on orders once the final order submission has been approved. Orders requiring less than 10 days will be assessed a minimum 50% surcharge.
- As of May 1, 2008, the counts for each member category are listed below. These counts are subject to change and are only used as an example. Members who have chosen not to be included in third party mailings are not reflected in the numbers.
